Service Description: Central location that receives, triages, and directs referrals to Diabetes Education Programs (DEPs), foot care services, addictions and mental health services through target="_blank">one-Link, and Hip & Knee Rapid Access Clinic (RAC), Low Back Pain RAC, and Shoulder Pain RAC

Services include:
  • single point of access for referrals to diabetes and mental health services within Mississauga Halton Region
  • musculoskeletal care for patients with moderate to severe hip and knee arthritis
  • facilitate timely referrals
  • link individuals to the most appropriate services
  • support best practice
  • promote collaboration 
  • leverage data for system planning and health outcome tracking
  • referrals to Halton Healthcare 3 hospitals (OTMH, MDH, GH) to expedite recommended endoscopic/colonoscopic investigations for patients who have already received an abnormal or positive Fecal Immunochemical Test (FIT) result
