Service Description: A community hub of service agencies sharing space to offer easier access to integrated health and social services * Care Connections partners include

Participating agencies include:
  • Acclaim Health, 1-800-387-7127: Services include hospice and bereavement support * Alzheimer services * nursing, personal support and companions * volunteer visiting * palliative care consultation
  • Food for Life, 905-873-6502: A food recovery program that collects surplus fresh and nutritious food from retail and wholesale suppliers and redistributes it to those in need via partner agencies throughout Halton
  • Halton ADAPT, 905-639-6537 ext 0: free counselling and support for alcohol, drug and gambling problems
  • Halton Diabetes Program (HDP), 1-844-739-0020: Provides diabetes education and clinical care for adults with prediabetes, type 1, or, type 2 diabetes
  • Halton Youth Justice - Ministry of Children and Youth Services, 1-800-461-5876: support, case management, and probation services for youth 12 to 17 years in conflict with the law
  • HMC Connections, 905-842-2486: Helping newcomers settle and integrate into community
  • Links2Care: 905-873-6502: Various community services, programs and supports for seniors and adults with disabilities, children and their families
  • Paramed: 1-800-465-5054: provides a full range of home health care services * community nursing clinic providing nursing services such as wound care, drain care, IV therapy, injections, catheter care, lymphedema care, etc.
  • Thrive Counselling: 905-845-3811: Family counselling agency providing counselling services for individuals, couples, and family
