Service Description:

Medical care team offering direct access to a multidisciplinary healthcare services * improves access to health care services * team includes family doctors, registered nurses, nurse practitioners, social workers, registered dietitians, pharmacists, physiotherapist, and chiropodist- health promotion
- disease prevention
- nutritional services (Registered dietitians provide nutrition counselling and education to enrolled patients)
- procedure clinic (offers once a month for assessment and performance of minor procedures)
- preventative care (breast cancer screening, cervical cancer screening, colorectal cancer screening)
- chronic disease management such as diabetes management program, smoking cessation, lung health program, etc.
- family health team pharmacists work directly with patients to ensure medications are being used safely and effectively
- immunizations (do not provide travel vaccines)
- mental health (may include counselling, lifestyle advice, medication or referral to a community program or specialist)
- palliative care
Family Health Teams are primary care organizations that include a team of interdisciplinary healthcare professionals who work together to provide comprehensive primary care to their patients
