Service Description: Community-based supportive housing. Target group is individuals, 18 years of age and older, who are living with severe mental illness. Self-contained bachelor apartments are offered along with a partial meal program, social/recreational activities, and professional support as needed. Referrals must be made through The Access Point by a community mental health professional. Those referred must have some ability to live independently and manage their own medications. Residents are encouraged to take an active role in the community.


Application: Referrals are through The Access Point. Please call or fill out the application form online. Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment.
