Service Description: Provides outpatient care, education and support to children and adults with type 1 and 2 diabetes, prediabetes and gestational diabetes * team consists of diabetes educators (nurse educators and dietitian educators), social worker, kinesiologist (exercise specialist), endocrinologists, nurse practitioner, and support staff


Adult Education Program
  • one-to-one teaching and group sessions
  • initial individual assessment
  • follow-up visits
  • one-to-one appointment a Nurse Practitioner in Endocrinology or an endocrinologist
  • all types of diabetes and prediabetes

Fees: Free
Application: Primary care providers (family doctor or nurse practitioner) or community endocrinologists can refer a patient by contacting the Mississauga Halton Central Intake Program * adults interested in attending a Diabetes Education Program can also contact the Mississauga Halton Central Intake Program at 905-338-2983 ext 4872 to initiate the referral process (self-referral)
Eligibility / Target Population: Adults 18 years and older with pre-diabetes, type 1 diabetes and type 2 diabetes * paediatric (5-18 years old, seeing at Oakville Location only)
