Service Description:

Medical care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors, psychologist, registered nurses, nurse practitioners, mental health workers, dietitian, health educator, speech language pathologist, pharmacists and chiropodist
Telephone Health Advisory Service (THAS) is available to Halton Hills Family Health Team enrolled patients when clinics are not open * patients who call THAS have access to a Registered Nurse who will provide them with symptom-based health advice and information
