Medical family health team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes 6 family doctors, 2 nurse practitioners, 3 registered nurses, an RPN, a mental health counselor and a pharmacist- memory clinic
- free seasonal flu shots by appointment
- medical lab by appointment only * call the lab from the parking lot at 905-842-7154 ext 242 when coming in for blood work
- acute care
- normal development, prevention and screening * care of healthy people, detection of conditions which may not be apparent yet
- palliative care
Practice focused on fully coordinated, comprehensive and continuity of care through preventive care measures, chronic disease management, mental health counselling, integration with Ontario Health atHome Mississauga Halton, electronic medical record, availability for same day appointments (Advanced Access), coordination and integration with pharmacist
Chronic Disease Prevention and Management Program - Sessions to learn ongoing self-care * includes information about coping, health guidelines, medication usage, working with health professionals, and informed treatment decisions
- Services are provided at the office, at home or hospice (palliative care clients), at the Oakville Trafalgar Memorial Hospital (OTMH), at retirement homes, and at Post Inn Village Long Term Care * family practice services include acute episodic care, chronic disease management, normal development prevention and screening, and palliative care * long term care patients, admitted patients and palliative clients (at home or hospice) have access to their family physicians through Dorval Medical 24/7
- Family Health Teams are primary care organizations that include a team of interdisciplinary healthcare professionals who work together to provide comprehensive primary care to their patients
