Nurse Practitioner-led multidisciplinary clinic providing safe, accessible and gender affirming care within a multidisciplinary, shared care model * team includes a Registered Nurse (RN), Registered Practical Nurse (RPN), Social Worker, System Navigator, and Pharmacist
Services include:
- support for social, medical and legal transition processes
- education and information on gender identity and healthcare options
- surgical assessments, referrals and support
- sexual health education and screening
- individualized support for hormone therapy and monitoring
- psychosocial support throughout the transition process
- group therapy sessions and workshops
