Service Description:

A collaborative community outreach project which provides the community of Weston and Mount Dennis neighbourhood with access to the following programs and services:
- Mental and physical health & wellness programs
- Youth, seniors, and diversity programs
- Employment and housing support
- Education and learning opportunities
- Social and recreation programs
- 12-week intensive case management available
- Support transitioning from hospital to home
