Service Description:
|
Supportive housing for adults who struggle with mental health and substance use concerns. Staff work with tenants on issues of physical and mental health, reducing harm around substance use and helping them to navigate systemic barriers of stigma and discrimination.

Application:

Referrals are accepted from anyone, including self-referrals.
Referrals are through The Access Point. Please call or fill out the application form online.
Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment.
