Service Description:
|
On-site housing support services for individuals living with serious and persistent mental health needs and history or risk of homelessness. Community Mental Health Workers provide direct skills teaching, supportive counselling, liaison with other community resources, goal-oriented individual functional and resource assessment, advocacy, social and recreational events and crisis prevention and intervention.

Referrals are accepted from anyone, including self-referrals.
Referrals are through The Access Point. Please call or fill out the application form online.
Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment.
