Service Description:

Intensive case management services for women and men living with HIV/AIDS, and with substance use and/or mental health challenges.

Application:

Referrals are accepted from anyone, including self-referrals.
Referrals are through The Access Point. Please call or fill out the application form online.
Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment.
