Service Description: A program / location of Oak Park Neighbourhood Centre, offering supports to the immediate community and bringing together multiple agencies to provide full wrap-around supports to people struggling with addiction, mental health, housing and low income to reduce overdose, move unhoused into housing and provide support to get people back on their feet with dignity. The program provides:
  • Harm reduction supplies
  • Nurse Practitioner primary care support
  • Addicitions & Mental Health Counselling
  • Problem gambling, financial and life skills, substance use concerns
  • Tax completion
  • Utility support
  • Drop-in programs
  • Goal setting
  • Form completion
  • ID
