Service Description:

Halton Region providesone-time financial assistance to help with the cost of funerals and health needs, including select assistive devices recognized by the href="https://www.ontario.ca/page/assistive-devices-program" target="_blank">Assistive Devices Program
- eligibility criteria varies across different supports
- for many benefits, documentation from a medical professional or vendor is required to confirm either need and/or cost
- do not order or pay for a service or item prior to approval from Halton Region staff as you will not be reimbursed for anything purchased prior to approval
Health-related benefits and medical equipment
May cover a range of supports (with required assessments, referrals, and cost estimates), including:
- Adult day programs (referral required through Ontario Health atHome)
- Baby layettes and medically necessary infant equipment
- Bath aids and other basic assistive devices for safety and daily living
- Custom footwear for severe foot deformities
- Emergency dental services for adults aged 18–64 (pre-approval required)
- Eyeglasses through approved vendors
- Hospital beds, lifts (mechanical, ceiling, Hoyer), and other mobility-related equipment
- One-time prescription medication coverage (subject to limits and eligibility)
- Basic and essential medical equipment required for health and safety
- Orthotics (one pair, within funding limits)
Assistive Devices Program top-up coverage
The following items are partially covered under the Provincial Assistive Devices Program. Halton Region may assist eligible residents with low incomes with their portion of the cost not covered by the Assistive Devices Program
- Hearing aids (assessment and estimate required; excludes ongoing supplies like batteries)
- Mobility aids (based on medical assessment)
- Prosthetics and related necessary items
- CPAP (sleep apnea) machines and related supplies (periodic replacement limits apply)
Funerals The Region provides financial assistance on behalf of deceased Halton residents whose estates cannot cover the cost of a funeral
-Two levels of support available:- Basic funeral support: simple burial or cremation with limited service
- Traditional funeral support: includes visitation and funeral service
- Amount of funding depends on the financial circumstances of the deceased;

- proof of address (for example, a driver’s licence, utility bill, other bills or mail addressed to you);
- proof of income (for example, your most recent Notice of Assessment);
- proof of status in Canada (for example, your birth certificate, passport or record of landing); and
- proof of a government-issued photo ID (for example, your driver's licence, Ontario Photo Card or passport)

Eligibility / Target Population:
|
Applicants must:
- be residents of Halton
- have a total net annual household income that falls below the Net Low-income Cut-off (LICO) for Halton Region
- include valid government-issued identification for each household member in the benefit request (birth certificates and immigration documents)
- give verbal consent to staff to record information in order to assess eligibility and issue benefits
