Service Description:
|
The HOST program is designed for adults experiencing housing instability and complex mental health needs within the context of broader systemic barriers. The program recognizes that factors such as health, aging, and substance use can intersect and shape each person’s experience.
The program offers rent-geared-to-income housing alongside mental health supports. Individuals in the HOST program work closely with a community mental health worker who ensure they are provided with the support to maintain their housing and improve their quality of life.

Referrals are accepted from anyone, including self-referrals.
Referrals are through The Access Point. Please call or fill out the application form online.
Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment.
