Service Description: Provides support, education and consultation to people and family affected by Alzheimer's disease and dementia

Services offered:
  • Adult Day Program - two locations: Oakville (Sheridan College, K-Wing, 1430 Trafalgar Rd, Mon-Fri 8am-5pm, Sat 8:30am-5pm); and Burlington (2180 Itabashi Way, Mon-Fri 8:30am-5pm)
  • one-on-one caregiver consultations in person, over the phone or via e-mail
  • caregiver education sessions - dementia information series * mid to late stages of dementia discussion series * presents info on planning for the future and preparing for transition into long-term care for Alzheimer family caregivers (7-9pm)
  • caregiver support groups - spousal, family and long term care support groups offered at various locations across Halton * see details

  • Memory Visiting Program - visiting volunteers provide companionship and physical activity for people experiencing memory loss and the early stages of dementia
