Service Description: Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible 

Services include:

Alzheimer Services:

  • one on one caregiver support
  • caregiver education sessions
  • adult day program (2 locations: Burlington and Oakville)
  • memory care (respite, home safety assessment, on-going assessment of care needs)
  • memory visiting (trained volunteers provide companionship, some respite)

Home Health Care:
  • nursing (visiting, shift, specialty, and clinics)
  • personal care
  • help with getting to appointments, errands, grocery shopping
  • Philips Lifeline (personal medical alarm and response)

Volunteer Visiting
:
  • friendly visiting
  • bereavement peer support program
  • hospice volunteer visiting
  • ongoing support groups, walking groups
  • hospice volunteer Visiting (trained volunteers provide emotional and social support to people with life-threatening illnesses)
  • home support exercise program
  • memory visiting
  • Tele-Touch

Palliative Care Consultation
:
  • education
  • mentoring
  • resources
 
Hospice and Bereavement Support - Companionship, respite care, and emotional support to people and their family/caregivers dealing with a life-threatening illness * services provided at home
 
Tele-Touch - A telephone reassurance program where volunteer callers phone lonely, isolated or frail older adults and adults with limited mobility, providing friendly conversation, security and reassurance * volunteers call according to a pre-arranged schedule * presentations to community groups are available
